Intensive Community Treatment for Older People
What we do
We provide community-based treatment to older people within the Aged Persons Mental Health Program catchment area, as an alternative to treatment in hospital during an acute phase of mental illness. By providing treatment in the person's home or usual place of residence, we aim to prevent hospitalisation where possible, or minimise the length of an inpatient stay.
Eligibility
Any older person within the Aged Persons Mental Health Program catchment area who is identified as requiring acute mental health treatment for which home care is assessed as a safe and acceptable option is eligible for our service.
We do not replace the need for community nursing services, for functions such as medication administration.
Frequency
We provide intensive services, at least daily and more than APATT can provide. Active treatment has to be required for us to become involved.
